Your patient. Your treatment plan. A different ward.
Mediclinic At Home lets you admit an eligible, low-acuity patient to hospital-level care in their own home. You remain the treating doctor: you prescribe, you review, you discharge. We provide the monitoring, nursing, coordination and escalation that make it safe.
What it is
Acute, hospital-substitutive care at general-ward level. Admission Avoidance (from the Emergency Centre or doctor's rooms, a full substitute for admission) and Early Supported Discharge (continuation of an in-hospital stay). Standard stream (self-administered treatment, once-daily nurse visit, Early Supported Discharge only) and Comprehensive stream (parenteral treatment, one to three daily nurse visits per the treatment plan, always used for Admission Avoidance).
Your professional position
Participation is voluntary and does not affect hospital admitting privileges, specialist relationships or patient allocation; the treating practitioner keeps responsibility for diagnosis, treatment, prescribing and discharge; clinical and funding decisions are separate, and schemes set funding rules; you are not expected to be available 24 hours (cross-cover, locums, escalation pathways and the Medical Officer on call); notify your indemnity provider.
Who it's for
Inclusion: general-ward level care, adult 18+, active member of a participating scheme, within 30km by road of a participating hospital, support at home, running water, electricity and ablution facilities. Exclusion: unsafe area, uncontrolled psychiatric condition, undifferentiated diagnosis, more than three one-hour nursing visits a day, IV opiates. Criteria are not prescriptive and admission remains at the doctor's discretion.
How it works
Referral pathway: assess and offer → submit admission application and prescription chart; nurse obtains consent → Care Coordinator validates, obtains scheme authorisation and arranges dispensing → patient discharged home → nurse sets up devices and starts treatment → daily virtual consults via the clinician dashboard → discharge decision with the Care Coordinator → 48-hour and 30-day follow-up.
Care team and escalation
24-hour remote monitoring by professional nurses; homecare nursing; clinician dashboard (trended vitals, clinical records, video consults); Medical Officer on call; ER24 failover; on request: physiotherapy, pathology, radiology, home oxygen, 24-hour care worker (subject to authorisation).
Hours
Scheme pre-authorisation 07:00–18:00 daily (outside this window admit to hospital; Early Supported Discharge may follow next day). Home nursing 06:00–22:00 daily. Care Collaboration Centre 24 hours.
Funding
Paid from the scheme's Hospital at Home risk benefit; full basket of in-hospital benefits. Participating schemes list. Named plans where a co-payment may apply if the patient opts for hospital, plus guidance on discussing funding with the patient.
Refer a patient
At launch: call the Care Collaboration Centre or email the admission application and prescription chart. State what information is needed and what happens after referral. A secure online referral tool is a later phase.
